GPPC5001: Primary Health Care Fundamentals

2024 unit information

This unit aims to introduce clinicians to managing the health of the community in primary health care environments. Primary care models and settings will be discussed. Concepts of shared decision making, collaborative and team care models, and clinical reasoning in managing chronic conditions will be explored. Health priority areas and public health approaches will be introduced. Co-ordination and context of practice environments will also be covered, as will doctor and professional well-being.

At the completion of this unit, you should be able to:

  • LO1. Discuss various definitions and concepts of health and what factors influence the health of an individual
  • LO2. Describe the components of a health system and apply the WHO model to describe the Australian and international health systems
  • LO3. Explain the benefits for national health outcomes of a strong primary health care sector using supporting literature
  • LO4. Use examples from the Australian primary care sector to explain current challenges and innovative solutions
  • LO5. Examine the unique model of health care delivery in the primary health care sector
  • LO6. Describe the benefits of patient centred, continuous, coordinated, comprehensive health care
  • LO7. Explain the patterns of clinical reasoning and predict sources of bias which may lead to clinical errors
  • LO8. Demonstrate how quality and safety can be implemented at various levels of health care delivery to improve patient health outcomes
  • LO9. Describe the roles of the primary care practitioner
  • LO10. Describe the fitness to practice and consider the challenges of treating colleagues
