This unit consolidates pharmacists’ ability to make safe, person-centred and evidence-informed prescribing decisions in the management of chronic conditions associated with cardiovascular risk reduction. Learners will develop advanced knowledge of the signs, symptoms, diagnostic features and management of dyslipidaemia, hypertension and type 2 diabetes mellitus. Using these primary care conditions as the clinical context, learners will apply the four-step prescribing process of information gathering, clinical decision-making, communicating and documenting treatment plans, and monitoring treatment responses. The unit further develops the cognitive and clinical skills required for prescribing, fostering knowledge and transferable capabilities applicable across practice settings and prescribing models. Learners will expand their understanding of the principles underpinning safe prescribing practice, including quality use of medicines, person-centred care, shared decision-making, clinical governance and interprofessional collaborative care. They will further develop their understanding of professional scope of practice to support safe, legal and ethical prescribing decisions. As part of the unit, learners will undertake workplace-integrated learning under the supervision of a Designated Prescriber (DP), an Ahpra-registered health professional with prescribing authority and relevant clinical experience. Through supervised practice, mentorship and competency-based assessment, learners will apply and further develop their prescribing knowledge and skills in an authentic clinical environment.
