Implementation of the Choose to Move program in Sydney, Australia

Choose to Move (CTM) is a 3-month program that provides motivation and support to adults aged 50 and over to become more physically active and socially connected. CTM was designed in Canada and has shown to successfully increase physical activity, mobility and reduce social isolation in older adults.

The program includes 8 group sessions where participants will learn about a range of health topics which may include physical activity, incidental activity, goal setting, nutrition, fall prevention, and other topics depending on the group's needs. There is also a one-on-one session with an Activity Coach to discuss physical activity goals and creating a personalised physical activity plan.

Participants will be expected to attend the group sessions in-person and complete surveys and physical activity measurements at baseline, 3 months and 6 months.
